Transition Care Intelligence LLC ("TCIQ", "we", "us", or "our") operates GoTCIQ.com and the TCIQ care coordination platform. This Privacy Policy describes how we collect, use, and protect information in connection with our services.
Information We Collect
We collect information in two contexts: from skilled nursing facility (SNF) operators who use our platform, and from patients and their family contacts who receive post-discharge outreach on behalf of those facilities.
- Facility staff: Name, email address, job title, and facility affiliation provided during account setup.
- Patients and family contacts: Name, mobile phone number, and health-related information shared during AI-assisted post-discharge outreach calls and SMS interactions.
- EHR data: Demographic, diagnosis, medication, and care plan information received via FHIR integration with facility EHR systems, used solely to support discharge workflow and outreach.
- Usage data: Log data, IP addresses, and browser information collected automatically when you use our platform or website.
How We Use Information
- To conduct post-discharge follow-up outreach via SMS and voice call on behalf of participating SNF facilities
- To administer CoreQ Short Stay Discharge satisfaction surveys and submit results to AHCA LTC Trend Tracker
- To flag clinical concerns to designated facility escalation contacts
- To provide leadership analytics and reporting to facility operators
- To operate, maintain, and improve the TCIQ platform
CoreQ Short Stay Discharge satisfaction survey responses — including resident name, discharge date, facility, and coordinator — are submitted to the AHCA LTC Trend Tracker quality registry on behalf of participating facilities as part of their quality reporting obligations. This submission is conducted under the facility's direction as the covered entity.
We do not sell, rent, or share personal information or protected health information with third parties for marketing purposes.
Outreach Communications
Phone numbers collected through our platform are used exclusively to deliver post-discharge care follow-up and satisfaction survey messages on behalf of the patient's skilled nursing facility. Message frequency is typically 2–4 messages per discharge episode, including 48-hour and 7-day follow-up and satisfaction survey delivery. Messages may include embedded links to surveys.
To opt out of SMS messages at any time, reply STOP to any message you receive. To request help, reply HELP or contact us at [email protected]. Message and data rates may apply.
You may also receive AI-assisted voice calls as part of post-discharge follow-up. To opt out of voice calls, reply STOP to any SMS message you have received, or contact us at [email protected]. Opt-out applies to both SMS and voice outreach.
Protected Health Information (PHI)
TCIQ operates as a Business Associate under HIPAA in connection with participating SNF facilities. Protected health information is handled in accordance with applicable HIPAA Privacy and Security Rules. We maintain a Business Associate Agreement (BAA) with each covered facility. PHI is stored on HIPAA-eligible Microsoft Azure infrastructure with database-per-tenant isolation and 6-year audit log retention.
Third-Party Service Providers
We work with the following sub-processors to deliver our services. Each is engaged under appropriate data processing or business associate agreements:
- Microsoft Azure — Cloud infrastructure and data storage (HIPAA-eligible, BAA in place)
- Twilio — SMS and voice call delivery. A Business Associate Agreement will be executed with Twilio prior to any protected health information or patient contact data being transmitted through their services.
- Anthropic — AI language model for outreach message generation. Patient data passed to Anthropic's API is de-identified and contains no protected health information. Anthropic is not engaged as a HIPAA Business Associate.
Data Retention
PHI and patient interaction data is retained for a minimum of 6 years in compliance with HIPAA requirements. Facility staff account data is retained for the duration of the facility's subscription and for a reasonable period thereafter. You may request deletion of non-PHI personal data by contacting us.
Your Rights
Depending on your jurisdiction, you may have rights to access, correct, or delete personal information we hold about you. Patients wishing to exercise rights related to their PHI should contact their SNF facility directly, as the facility is the covered entity under HIPAA. Changes to This Policy
We may update this Privacy Policy from time to time. We will post the updated policy at this URL with a revised effective date. Continued use of our services after a change constitutes acceptance of the updated policy.
